The first step towards a beautiful, healthy smile is to schedule an appointment. Contact our office by phone 403-457-2233 or complete the appointment request form. Our administrative staff will contact you to confirm your appointment.
 

Patient Registration

You may preregister with our office by filling out our Patient Registration Form. After you have completed the form, please make sure to send us your information via e-mail: acedental12@gmail.com 

On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it. Another option is to print the registration form, fill it out in writing, and bring it during first appointment.
 

Your First Visit

Your initial appointment will consist of an examination and review of your treatment options. Usually, treatment can be done or started the same day as the consultation. However, complex treatment plans will require an evaluation and a second appointment to provide treatment on another day.

 

Please assist Ace Dental by providing the following at the time of your consultation:

  • Any x-rays taken by a previous dentist, if applicable.

  • A list of medications you are presently taking.

  • If you have dental insurance, bring the necessary completed forms

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office by mail,  e-mail, or bring them in person. If additional x-rays are necessary, they can be taken at our facility.
